To book an appointment:

Please follow the steps below to book an initial consultation or follow-up appointment with me.

Step 1: Choose an appointment time

You will receive an email confirmation and have the option to add the appointment to your calendar.

Step 2: Complete my waiver & informed consent (required for new clients)

This form is required for all new clients prior to your first consultation.

Step 3: Send me test results (optional)

Please send up to three test results (e.g. stool/microbiome test, your most recent basic blood panel, and possibly one other stool or relevant test) in a single email to [email protected]. These will be automatically added to my files for your case.

Note: For Thorne Gut Health Test results, please send me both a PDF of the consumer report (use File -> Print to PDF), and the raw data, which can be downloaded as a .csv file (under the “Test Results” tab, click “Download” on the top right, then select “Full Microbe Community”)

Step 4: Show up to your appointment

You’re all set! I’ll either meet you at the Zoom link provided in your email confirmation or call you at your appointment time.
